I went through your query.

I have reviewed your reports (the attachments were removed to protect the patient's identity). The treadmill test (TMT) is not attached. However, other reports are normal except for deranged LDL (low-density lipoproteins) cholesterol. Considering your age, symptoms, and all normal reports, these symptoms are unlikely to be due to cardiac causes. Deranged LDL cholesterol will not cause any symptoms. However, it is a future risk factor for coronary artery disease. So, you should have a healthy lifestyle, like avoiding fatty, fried, and oily foods, and you should do regular exercise. Regarding possible causes, anxiety is one of the causes that can cause awareness of one's heartbeat and palpitations. Gastric reflux can also be a cause, especially if you have other associated gastric symptoms like bloating, burping, nausea, etc. Also, it would help if you got your Vitamin D level tested, which can cause musculoskeletal chest pain. Regular exercises will increase the vagal tone and reduce the resting heart rate. You can continue your current medicines and take antacids for a week. Kindly consult a specialist, talk with them, and take medicines with their consent.

Your TMT (treadmill test) is also normal and rules out the cardiac cause for your symptoms. Gastric reflux may be responsible for your symptoms, and besides antacids, you should also avoid oily and spicy foods, avoid heavy meals, and take some walks after each meal.

Your lipid profile, especially LDL cholesterol (attachment removed to protect the patient's identity) is better than the last result. However, it is still slightly elevated. So, you need to continue a healthy lifestyle on a long-term basis. However, even being slightly elevated, it will not cause any symptoms, but it will increase the risk of CAD in the future. At the moment, if there is further scope for lifestyle improvement like regular physical activities, and avoiding oily and fatty diets then you should try to focus on that. If you think that you will not be able to improve your lifestyle further then better to take the six weeks course of lipid-lowering medicine like Atorvastatin or Rosuvastatin 10 mg. So, you can get it prescribed by a local doctor. However for maintaining cholesterol under control, you need to follow a healthy lifestyle, or else it will rise again. Additionally, first-degree relatives should also be tested for lipid profile (if not done in the recent past).

I have had a viral cough, cold, and fever for the last three days, and the doctor has prescribed me Oseltamivir 75 mg, Alegra M, and Dolo 650 for five days. However, I have read that Oseltamivir has serious side effects, such as sudden cardiac death or irregular heartbeat. So is it safe to take as I have had heart palpitations for the last two months and am on Ciplar LA 40?

According to me, you can start tapering it now. You can have 20 mg and 10 mg for two weeks and then stop. If you stop it suddenly, then you can have rebound palpitations. You should continue to have regular physical activities and meditation as well to keep palpitations under control.

Your lipid profile report is acceptable now (attachment removed for patient identity protection). The most crucial component is LDL cholesterol, which has decreased from 146 to 124 mg/dL. The acceptable range is below 130 mg/dL. However, less than 100 mg/dL is further beneficial. Although this is reasonable control, if you can continue a healthy lifestyle and regular physical activities, it can come down further. Triglycerides are slightly up but in a normal range, and HDL (high-density lipoprotein) is down. So, the lipid profile has improved, and we should continue with a healthy lifestyle to maintain it and further reduce LDL (low-density lipoprotein) if possible.
