Hi doctor,

For three months, I have been experiencing chest pain. Before two months, I did ECG, echo, and TMT, and the results were negative. Now again, I am having chest pain along with left-hand pain, back neck pain, and stomach burning. A burning sensation spreads to the heart and even the throat from the stomach. What could be the reason for this? Is it angina or any other heart-related issues? Please advise on this. Thank you.

I understand your concern. You need to take an electrocardiogram (ECG), even if all tests were normal. There can be possible peptic ulcer disease (ulcer in the stomach lining), but we need to rule out cardiac causes in every chest pain. Then you can consult with a gastroenterologist for gastritis. It is better if you can upload a recent electrocardiogram (ECG) with previous echocardiogram and treadmill test reports. Thank you.

I have gone through all of your reports (attachment removed to protect the patient's identity) (electrocardiogram, echocardiogram, and treadmill test). Your recent electrocardiogram (ECG) is normal. Only sinus tachycardia (heart rate is slightly on a higher side of 101). But there are no signs of ischemia or coronary artery disease (CAD). Your echocardiogram test reports are also normal, but mild mitral valve prolapse with mild mitral regurgitation, which is a structural abnormality but not significant, is seen. It may cause increased heart rate or palpitations. It may or may not progress further, but I suggest you do a yearly echocardiogram review to see its progression. Treadmill test results (TMT) are also normal, which concludes even with exercises, there are no signs of ischemia or coronary artery disease (CAD). So your pain does not seem to be angina. It can be non-cardiac muscular pain or secondary to anxiety. If it is an angina condition, chest and neck pain might or might not radiate to any arm, increasing with walking or exertion. So if chest pain increases on exertion, walking, or jogging and relieves with rest, that can be angina. I would suggest you do the following. If you have palpitation or tachycardia (heart rate more than 100), you can start taking tablet Metoprolol Succinate 12.5 mg once daily (consult with a specialist doctor, talk to him or her, and take medicine only with their consent) or consult a cardiologist in person for same issues. Do exercise or take a brisk walk for 30 minutes every day. Avoid tobacco products, smoking, etc. If anxiety or panic attacks are present, get treatment for the same. Thank you.

Your ECG is not showing any signs of heart attack or ischemia. In this ECG interpretation, a borderline first-degree AV (atrioventricular) block is mentioned. Explanation - PR interval is prolonged in some leads which I have seen in your previous ECG too. So it is not an exact 1st degree AV block and no need to worry. So have you started any drug to control the rate because your heart rate is normal? For chest pain again, you can do a TMT (treadmill test). Mostly it will be negative (normal) but for confirmation, please correlate your chest pain symptoms whether these are related to anxiety or panic. It is not a heart attack as per ECG. Some blood tests like CPK-MB (Creatine kinase myocardial band) and Troponin can also be done while chest pain.

I am not taking any tablets. Please confirm do I need to take all the tests immediately like TMT, echocardiography, and troponin.

No. If you are having chest pain, just do troponin -T or I (whichever is available). If it comes normal, then just plan TMT for any day. If troponin is raised then we can suspect ischemia.
