Can my stage 3 CKD be managed without dialysis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 52-year-old and was recently told I have stage three CKD. My creatinine stays around 2.1, and my GFR is 40. I feel tired all the time, and swelling in my feet is becoming frequent. I also have type 2 diabetes and hypertension, so I guess that is the reason.

  • I want to know if diet changes like low-protein or avoiding salt can really slow the damage.

  • I want to know whether dialysis is inevitable for me, or if CKD can be managed long-term without it?

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

Stage 3 chronic kidney disease (CKD) at age 52, with an estimated glomerular filtration rate of 40, means your kidneys are moderately reduced in function, but dialysis is not inevitable at this stage. Many people with stage 3 CKD remain stable for years with careful management.

  • The most important steps are tight control of blood sugar and blood pressure, since diabetes and hypertension are the leading causes of CKD progression; your doctor may optimize medications such as angiotensin receptor blockers, which also protect the kidneys.

  • Diet does play a role: modest protein restriction (around 0.27 to 0.36 grams per pound per day) can help reduce the kidneys’ workload, but eliminating protein is not advised. High-quality sources like eggs, lean meat, or dairy in controlled portions are recommended.

  • Limiting salt is important to reduce swelling and control blood pressure.

  • Potassium and phosphorus may need monitoring depending on lab results, but foods like bananas or dairy are usually fine in moderation unless blood levels are elevated.

  • Lifestyle factors like maintaining a healthy weight, regular exercise, avoiding NSAIDs (nonsteroidal anti-inflammatory drugs), and not smoking also help slow progression.

  • Swelling in the feet can often be managed with salt restriction and diuretics, and fatigue may improve once anemia, common in CKD, is addressed.

With regular follow-up, medication optimization, dietary guidance from a renal dietitian, and careful monitoring, many people with stage 3 CKD maintain stable kidney function for years without needing dialysis, so the focus now is on slowing progression and managing complications.
