Hi doctor,

I am a 40-year-old male. I am suffering from throat irritations for the last three months. I feel like something got stuck in my throat and I feel like phlegm, but it is not coming out. I am not using pillows when I sleep, if I use pillows I am feeling stiffness in my throat. I have no swallowing difficulties until now. Currently, I have no fever and cough. I have recurrent tonsillitis. If I drink cold water then the next day I will get throat pain. Can you please give me some advice?

I can understand your problem. You may be suffering from chronic pharyngitis with tonsillitis. This chronic pharyngitis may lead to irritation in the throat and foreign body sensation.

Other possibilities like acid reflux, or postnasal discharge, or sleep apnea may also present like this.

The Probable causes

Allergy or cool drinks intake or exposure to dust or pollen.

Investigations to be done

Completed blood pictur., x-ray of paranasal sinuses, and RBS (random blood sugar).

Differential diagnosis

Acid reflux. Sleep apnea. Granular pharyngitis.

Probable diagnosis

Dysphagia under evaluation probably due to chronic pharyngitis.

Treatment plan

Tablet. Amoxicillin and Clavulanic acid 625 mg twice a day for five days after food, tablet. Azithromycin 500 mg first day followed by 250 mg once daily for four days after food, tablet. Pantoprazole 40 mg before breakfast for one week, and tablet. Levocetirizine twice daily for five days after food.

Preventive measures

Avoid cool drinks and ice creams. Drink warm water regularly. Use salt water gargling thrice a day for five days. Use steam inhalation thrice a day for five days.

Regarding follow up

Review after five days.
