Hello doctor,

My circumcision surgery was done a month ago. The swelling and surrounding area seemed to improve until last month. In the mid of last month when I was cleaning the area, I noticed it starting to burn around the bottom swelling area. During those couple of days, I saw the swelling lightly increase. It is now late and have not seen any drop of swelling and worried that this would never heal. Also, my tip area has some light swelling which has not changed for the past eight weeks (it was pretty bad for the first two weeks). My erection last month did not hurt, but now when I get erections, I have some discomfort due to the dryness or redness. I saw my doctor last week who did the surgery, but he did not say much. I am attaching photos on how it looked last month and how it looks now. The picture with the stitches is what it was last month. If you can help me answer the questions below, that will be great since I am stressing out. The lab tests conducted included a CBC blood and urine test, which showed normal results, as well as a urine test strip done last week, which also yielded a normal result. Additionally, the individual's blood pressure, blood sugar, and body temperature were found to be within normal ranges. Is it normal to see this type of swelling behavior? Is it normal to have swelling around the tip? Is this expected healing based on my current photos, and is it healthy to see swelling for three months? I started having a burning sensation, which is on and off after I cleaned the area. I am assuming this is just from the irritation that I caused. When I elevate my penis as I get out of the shower, the glands and swelling area look white or tan, like it is almost healed, and I see a slight decrease in swelling; is this a good sign? Can I have swelling from this great cause any long-term complications, or is there nothing I should be worried about? My glands looked dry and slightly red and cracked. Is applying sensitive lotion safe to this area, and will it help heal?

I have reviewed the attached image (attachment removed to protect the patient’s identity). Yes, it is reasonable to have this much swelling, which will resolve over time. Yes, it is usually expected to heal. Yes, the burning sensation is due to irritation. The appearance of tan following shower has nothing to do with healing. This is due to moisture following the shower. The swelling is unlikely to cause any long term complications. The cracks in the skin are due to dryness, and applying lotion is not harmful. However, you need to take the opinion of a dermatologist (skin specialist) if there is no improvement in six weeks.

Thank you for the reply.

I saw a dermatologist yesterday, and they advised me to apply Waxelene or one percent steroid cream for a week. I think I am going to use Waxelene for a couple of weeks. Is it safe to use the one percent steroid cream for a week? Will applying the Waxelene and touching or moving the swollen skin cause any complications, or is this considered safe? Does getting night erections delay the healing and cause any issues to the swollen skin in the long run? I have not seen any change in the swelling for the past six weeks, and I am assuming this is still normal. Should we give it some more time? From looking at my photos, does everything look normal and in line with the healing?

Do not worry. It is a normal healing process as far as swelling is concerned. Erection does not affect healing adversely. The application of steroid is not at all harmful for a short period. However, regarding cracks as well as dryness of skin, only dermatologist can provide an opinion.

Thank you for the reply.

I hope you are doing well. I am about seven months after the operation and wanted to check in regarding the surgery. I still have some discomfort and sensitivity, which I would assume will get better with time. How does it look so far, and is everything still in line with the healing process? How much longer would you expect for the ring swelling and tip swelling to go down? I spoke to a dermatologist regarding the dry skin, and she said you can apply Vaseline or Waxelene for the next two weeks to help. Photos are attached.

I am fine. Hope you are fine too. I have reviewed the pictures posted (attachment removed to protect the patient’s identity). The healing process is already complete. The swollen appearance is due to a folded prepuce, and the tip is normal. The sensitivity takes time to settle, and it is not an issue. Dryness is due to the nature of the skin and is unrelated to the surgery. Continue as per the dermatologist's advice. You need to follow the dermatologist's suggestions as I cannot opine on the matter of dryness. Let me reiterate, the appearance of your glans is normal, and you need not worry.

But the foreskin is not folded, and when I saw my primary care, he said it is swollen. My foreskin never looked like this, and it was smooth or flat. The ring swelling still looks the same as when we first spoke and is pulling my head, which is causing discomfort. Will I ever expect the ring swelling to go down, or is it going to remain like this forever?

That is not the swelling. It is the junction between the two folds of skin that are sutured together after excising the foreskin. That is going to remain like that and will remodel very little over years. In fact, that is the normal postoperative appearance, and I respectfully disagree with your primary care physician. I suggest you can ask the operating surgeon to have a look so that he can palpate and opine regarding the swelling, which I feel is non-existent by seeing pictures.
