Hello doctor,
My two-year-old grandson had a circumcision about seven weeks ago, but the area has not completely healed yet.
The lower part of the incision site is still swollen and appears bluish in color. I am concerned and would like to know if this is part of the normal healing process or if it needs to be checked by a doctor.

At around seven weeks after circumcision, the surgical area is usually expected to be mostly healed. Some mild swelling or sensitivity can occasionally take time to settle, but ongoing swelling with a bluish discoloration (a change in skin color that may happen due to blood trapped under the skin or bruising) at the lower part of the incision site is not something that should be ignored.
It would be best assessed by the surgeon who performed the procedure or a pediatric urologist (a doctor who specializes in urinary and genital problems in children).
There are several possible reasons for these changes, including:
Some children may have residual postoperative swelling or lingering swelling after surgery as the tissues continue to heal.
Scar tissue formation means healing tissue can sometimes become firm, raised, or slightly swollen as part of the normal scar-healing process.
Hematoma means a small collection of blood under the skin, which can cause a bluish or purple appearance.
Local inflammation, such as mild irritation or inflammation around the healing area, can delay recovery.
Adhesions occur when healing skin sticks to nearby tissues, which can sometimes affect the appearance of the area.
The fact that the swelling is limited to one area and appears bluish does not automatically mean there is a serious complication. However, because the healing process has taken longer than expected, it would be advisable to arrange an in-person examination within the next few days to make sure everything is healing properly.
In the meantime:
Keep the area clean and dry.
Avoid applying antiseptics, antibiotic creams, oils, or home remedies unless specifically recommended by the treating doctor.
Make sure your grandson is passing urine normally and does not seem uncomfortable while urinating.
Please seek urgent medical attention if you notice:
Fever or signs of illness.
Rapid increase in swelling.
Increasing redness, warmth, or pus-like discharge.
Severe pain or difficulty passing urine.
The penis becomes very dark, cold, or extremely swollen.
If possible, you may upload a clear photograph of the circumcision area. A photo can help assess whether the appearance is more consistent with normal healing, bruising (hematoma), scar tissue, or another issue that may need treatment.
