My niece has red islands or patches on the tongue surface. She is 23 months old. This has been there for the past one year. Please help.

These patches are related to vitamin B deficiency and gastric discomfort. Sometimes, they also have superadded infection of bacterial or fungal origin. I would advise you to start a multivitamin syrup, deworm the child, and apply mouth paint (Candida mouth paint) for four to five days.
The treatment plan includes:
Syrup A to Z (multivitamin), one teaspoon once daily for 30 days.
Syrup Zentel (Albendazole) two teaspoons single stat dose.
Enterogermina suspension (probiotic), one teaspoon twice daily for three days.
Candida mouth paint local application for four to five days.
