Patient's Query

Hello doctor,

I am an 18-year-old female. Last month I was seven days late on my periods followed by two days of bleeding, and I also experienced excruciating cramping. My regular cycle typically lasts five days. It was red, thick, and mucus-mixed blood, but after two days, it ended. The next day, I experienced a small amount of red spots. A week earlier, I had protected intercourse. I was unsure whether the bleeding was due to implantation, but after taking multiple pregnancy tests, all of the results were negative. The following month, which is at the moment, my period lasted three days, and I occasionally had extremely light pain that lasted only a few seconds. I had moderate sexual encounters around twice a week. In addition, I recently took a test, and it is still negative, so I am still waiting for my period. I am worried and depressed a lot. Is it possible for me to be pregnant, or is having two days simply a medical condition? I also want to mention that most of the time, condoms are on, but when they are not, he does not ejaculate within me.

Please help.

Hello,

Welcome to icliniq.com.

That is a lot of pregnancy tests, therefore you should probably stop. These cycle abnormalities might potentially manifest as

  1. A period date plus or minus seven days is typical; based on your photo (attachments removed to protect the patient's identity), it appears to be practically simultaneous. Since the hormonal axis between the brain and the ovary must be developed for a woman to have regular cycles, which happens at roughly 19 to 21 years of age, having a few irregular periods every so often is fine.

  2. You are under the normal weight range. You should weigh between 50 and 55 kg. So consume a lot of fruits, veggies, and healthy food. This healthy weight keeps your hormone levels in a regular range and ensures that your periods come at the proper time and frequency. So there is nothing to be done; simply wait for your periods to become regular.

  3. The possibility of vaginal infection is significant. Vaginal infection spreads during sex (so the partner is the cause), so it must be treated concurrently or you will get the infection the next time you have sex. You might experience a white, yellow, or grayish discharge with or without itching, an unpleasant odor, pain at the start of sex due to dryness (because lubrication will be reduced), and late in the session when the fully extended penis touches the innermost vagina, pain that radiates to the back and thighs. Bowel movement and gastric symptoms are both impacted by infection. The bleeding from periods will be heavier, darker, clot-like, or fleshy masses, may smell awful, and there may be more days of spotting after the period as well. The blood flow rises when the uterus is infected because the inner lining is inflamed, and the blood combined with the inner lining of the uterus creates a thick, doughy, or mushy mass. It maintains spotting or you have a discharge that is blood-colored because the lining's mending process is not complete. The majority of the time, an infection is not simply caused by yeast fungus but also includes a family of aerobic and anaerobic bacteria that cannot all be seen on culture. It will grow when the infection is treated and cleaned, and if it is not cured, it will come back. Ensure that you are not pregnant before therapy. I suggest you and your partner take the following medicines.

Tablet.Fluconazole 150 mg (antifungal) one tablet; once a day after dinner.

Tablet.Azithromycin 1 gm (antibiotic) one tablet; one hour before a meal.

Tablet.Secnidazole Forte (antibiotic) two tablets; after dinner.

Later, start taking a capsule of Doxycycline 100 mg (antibiotics) twice a day for one week, vaginal wash or V wash (Lactic acid and Sorbitol) or a similar cleanser twice a day, and a vaginal pessary - capsuleClingen forte or Candid -CL or Cansoft CL (antifungal) once a night for five nights. Your partner can use local application of any antibiotic or antifungal cream. For seven days, use (Candid B, Candiderma, or Lobate GM) on the penis twice a day after washing. Avoid engaging in sexual activities until the virus has completely cleared. Do not ignore the condition as delaying treatment may cause the illness to spread and harm both your and your partner's future fertility. So, depending on symptoms, determine the reason and treat it as appropriate.

Thank you
