Hi doctor,

I am 26 years old now. What is this redness around my nostrils, and how can I get rid of it? I have had it for more than ten years. This redness appeared when I was about 15 years old and has not gone away. I have tried creams and laser (for spider veins) and stopped wearing makeup. Nothing seems to make a difference. It is slightly more sensitive than the surrounding skin but does not hurt. I have been to a few dermatologists about it and was diagnosed with periorificial dermatitis, but the treatment they gave me did not do anything, if not make it worse. A dermatologist gave me a Clindatech lotion seven years ago after diagnosing it as periorificial dermatitis, but it did not have any effect.

Thanks for the response.

I have not noticed anything that aggravates it. It is always just constantly there. Maybe sleeping with makeup on (which I never do, only a few times in my life) and excess alcohol consumption has led to this condition. There is no photosensitivity or reaction to the sun. Spicy or hot foods or stress do not have an impact, and I have never used topical steroids or anything harsh on my face. No excess scratching or rubbing, I try not to touch my face, and I am particularly careful around my nose because I do not want anything to make it worse. If it is rosacea, is there a treatment that will make it go away? I do not have it anywhere else on my face or body.

Some suggestions for rosacea management:

  1. Avoid trigger factors, use a broad-spectrum sunscreen, cleanse with soap-free cleanser, use a gentle emollient, avoidance of harsh products (astringents, toners, menthol, camphor, and sodium lauryl sulfate), makeup (fragrance-based cosmetics).
  2. Apply 1 % Metronidazole gel or cream at night for 15 days.
  3. For redness, you can apply Brimonidine gel once daily for two weeks. First, we should try to control it with topical creams.
  4. If no response is seen, a short course of Doxycycline is suggested.
  5. Use sunscreen while on Doxycycline, as it is a photosensitive drug.
  6. Avoid direct sunlight. Always apply sunscreen. Use photostable sunscreen daily at 9 AM, 12 PM, and 3 PM. Also, apply 20 minutes before going outdoors. Sun protection is a must.
  7. Do not apply any harsh products to the skin. Use Cetaphil cleanser (cetyl alcohol and glycerine) to wash your face twice a day for two weeks.
  8. Tablet Antoxid HC (beta carotene and zinc sulfate monohydrate) once daily after breakfast for two weeks.
  9. Soapex soap (sodium palmate and glycerine) or Cetaphil soap for a bath for two weeks. Follow up after two weeks.

Hi doctor,

I have just come back from seeing a dermatologist who has confirmed the redness is rosacea, but she prescribed me Soolanta cream and Borymycin antibiotic for one month which I am hesitant to take for fear of making my gut issues worse. I went to see this dermatologist because I have been experiencing burning and extra redness on my skin near my nose. I have not been using any new products. I would love a second opinion on this treatment approach. She recommended I consider vascular laser therapy but that is very expensive.

  1. Apply 1 percent Brimonidine cream at daytime for 10 days (to reduce redness).
  2. Apply 10 percent Aziderm cream/gel at night for 5 days.
  3. Tablet Lim-o (vitamin C) once daily after food for 30 days.
  4. Increase vitamin A and C intake in your diet.
  5. No need of laser. Use a broad-spectrum sunscreen with good SPF factor.
