I have recently been diagnosed with dermatomyositis. I have attached my report. Please inspect and kindly help, doctor.

I read your query and can understand your concern. I reviewed your report (attachments removed to protect the patient's identity). We generally diagnose dermatomyositis with five things.

  1. Muscle weakness
  2. Elevated LDH (lactate dehydrogenase) and CPK (creatine phosphokinase).
  3. Myopathic EMG (electromyography)
  4. Muscle biopsy showing myositis.
  5. Skin changes consistent with dermatomyositis.

If you have all first four diagnoses, it is definite; three is a possible diagnosis, two is probable, and one is a doubtful diagnosis. However, you are on the proper treatment for dermatomyositis, and generally, one can say how good your response is only after four weeks.

Hello doctor,

Thank you for the reply.

I have muscle weakness, and it is terrible. My CPK (creatine phosphokinase) was high, for which I went for EMG (electromyography) and nerve conduction test. The EMG (electromyography) came back abnormal, suggesting myopathy. Thus the doctor asked me to go for a muscle biopsy, and it showed the results of the immune myopathy dermatitis pattern. Is there any way to improve my general feeling of being well other than medications, as I feel weak and in pain? My hands are very weak and I cannot lift my arms above my shoulders. What else could be helpful to relieve the symptoms? Kindly help.

I read your query and can understand your concern. The ideal way is to take a good diet containing citrus fruits, proteins, yoga relaxation, and exercise, but avoiding heavy exercise. Medicines can treat muscle inflammation. The initial 12 weeks are important. They will tell us about the response and recovery with medications, especially on high-dose steroids. Take enough water, around two to three liters but strictly follow medicines.
