I have been experiencing persistent loose motions for a month, despite taking Tiniba 500 mg (Tinidazole) which initially provided relief but the symptoms return after a few days. I would like to seek guidance on what steps to take next.

You have been experiencing diarrhea for a month, and a detailed workup is needed to determine the cause. The following investigations are recommended:

  1. Check for other abdominal symptoms such as blood in the stool, fever, abdominal pain, or bone pain.

  2. Stool routine microscopic examination needs to be done to analyze the stool for any abnormalities.

  3. A stool culture to identify any potential bacterial infections.

  4. Thyroid function tests (serum TSH and T4) and HIV testing by ELISA (enzyme-linked immunosorbent assay) to rule out any hormonal or infectious causes.

  5. If no cause is found, further investigations such as a colonoscopy may be necessary to examine the colon and take biopsy samples if needed.

Based on the test results, a management plan can be developed. If an infection is detected with the presence of pus cells and bacteria in the stool, an appropriate course of antibiotics will be prescribed. If parasite eggs are found in the stool, tablet Albendazole may be given. If the stool and other reports are normal, a colonoscopy with biopsy may be performed to investigate further. Stress-induced functional bowel problems should also be considered and discussed with a physician or gastroenterologist during the examination.
