Hello doctor,

I am a 27-year-old female. I have had diarrhea and loose bowels for over 20 years. I have been to a couple of gastroenterologists but no one took me seriously. I had a colonoscopy when I was 19 and the only thing they concluded was that my colon was inflamed, which was due to diarrhea for more than 14 years. I am passing food within eight hours. I know this is because whatever is in the bowel is undigested. I used to have around five to eight movements a day with less undigested food. But within the past one or two years it has become more common and I have two movements with more undigested food. I am getting more worried. I want to get this under control.

I read your query and understand your concern.

You may have irritable bowel syndrome. Have at least three small bottles of probiotic yogurt a day during breakfast, lunch, and dinner. Probiotics are live bacteria and yeasts that are good for your health, especially your digestive system. Probiotics are good or helpful bacteria in our gut that keeps the gut healthy. Probiotics are naturally found in our body but in patients suffering from constipation and other disorders, the natural gut flora is destroyed over time. Hence foods and supplements containing probiotics are to be used externally to replenish the activity of gut flora.

If you are experiencing excessive flatulence, please watch your fiber intake. Try to take a balanced fiber diet. Too much increase in fiber intake can cause excessive flatulence. As per your history, you are on a controlled diet and take five to seven servings of fruits and vegetables every day. Make sure that total fiber intake should not exceed 21 grams a day. Too much of anything is also not good. So fibers are good but again only to a limit. Two servings of fruits and vegetables will provide you with more than enough fiber required daily. You have to increase the amount of soluble fiber in your diet. Soluble fiber attracts water and turns to gel during digestion and adds a lot to the bulk of stool. This slows digestion. Soluble fiber is found in oat bran, barley, nuts, seeds, beans, lentils, and peas. It is also found in Psyllium, a common fiber supplement. So in case, your total soluble fiber intake is less you can take two teaspoons of Metamucil (Psyllium) or Fybogel (Ispaghula husk) once daily before going to bed. It will help to form the bulk of the stool and will act as an osmotic laxative also.

Apart from that, a few other things you can do are-

  1. Include prunes and dates in your diet.
  2. Drink a glass of lukewarm water daily half an hour before going to the washroom in the morning.
  3. Do not take bananas for the time being. Grapes, guavas, and pears are best for you.
  4. Try to take a little extra fat in your breakfast. This will increase your urge to defecate.
  5. Avoid all dairy products except yogurt. Buttermilk is a good source of probiotics due to the live cultures added to ferment the milk sugars. Cooking with it, however, will destroy the live cultures.
  6. Kimchi is a traditional Korean food manufactured by fermenting vegetables with probiotic lactic acid bacteria. Many bacteria are involved in the fermentation of kimchi, but LAB (lactic acid bacteria) becomes dominant while the putrefactive bacteria are suppressed during the salting of cabbage and the fermentation.
  7. Pickles are cucumbers that have been pickled in salty water and fermented. They are low in calories and high in vitamin K.
  8. Miso paste is a probiotic food that can help treat intestinal disorders. Miso paste is an Asian seasoning made by fermenting a mixture of soybeans, barley, brown rice, and several other grains with a fungus, Aspergillus oryzae. The result of this fermentation is a smooth-textured paste with a strong and salty flavor.
