Hi doctor,

I request your help in checking my blood, urine, and ultrasound reports. In the ultrasound report, it is written duplex moiety in the left kidney. I never observed that in any of my previous ultrasound reports. I need your advice regarding that. Other existing health issues for your reference: I am having the below problems regarding which I am taking gynecologist treatment: I got copper T inserted after that; I have had heavy periods. Three years back, I delivered a baby through normal delivery. After that, I have a problem while holding urine. Gynaecologist suggested it may be because of weakened muscles after normal delivery and told me to do some exercises to take care of that.

I understand your concern. The reports you have uploaded are pretty much normal. The only things slightly out of place are: Your hemoglobin (this can be due to your heavy vaginal bleeding), and Vitamin D levels - I will prescribe you some supplementation at the end. Coming to your ultrasound report, which shows Duplex left moiety - This is a fairly common condition where one of the kidneys is split into two. The most common way someone finds out is when they have a scan for an unrelated issue or if they have pain on the affected side. Ultrasound is not the investigation of choice to comment on duplex moiety accurately. All ultrasounds are user dependent and not 100 percent sensitive. You should not worry too much about it. Generally, it does not cause any problems for patients. I suggest you get a CT KUB scan if and when you have pain on the left side. There is no need for it at the moment if you do not have any problems. A CT scan will give the best picture of what is going on with your kidneys. Coming to your urinary symptoms (yes, it is common after vaginal delivery if your pelvic floor muscles are weakened).

I require a few more details so that I can assist you with it: Do you leak urine when you cough, sneeze, laugh or pick up something heavy? Do you have any urgency to pass urine, and does it leak before you can reach the toilet? What exercises did your gynecologist advise you to improve your pelvic floor muscles? Are you breastfeeding at the moment? These are the medications you need for your hemoglobin and vitamin D levels: Capsule D-rise 60K units - Once a week for eight weeks. - followed by once a month for one year, and tablet Orofer XT one tablet per day for three months.

Thank you for the reply.

I have never felt any pain in my left side, so I believe I don't need to get any CT done or don't need to take any action regarding Duplex Moiety. Is it not a serious thing? Regarding Urine: Yes, I leak urine when I cough or sneeze, but only in case I have high urine pressure; otherwise do not. Yes, I feel an urgency to pass urine, which sometimes leaks before I reach the toilet but mostly in the evening when I travel back home from the office.

No, you do not need to do anything about your duplex moiety now if you do not have any symptoms. However, if you ever do, please get a CT scan. As far as your urinary symptoms are concerned - please do kegel exercises three to four times a day. This will eventually strengthen your pelvic floors. I suggest you take this medication (in case you are not breastfeeding at the moment) tablet Mirago 50 mg 1-0-0 before breakfast in the morning for one month. This will help reduce the urgency but may not work for the leaks due to coughing or sneezing. This medication does cause constipation in some patients, so please take a stool softener syrup if it does. Do not hold urine for long periods and pass urine as soon as you have the sensation to do so. Please review it in 15 days to let me know if the tablet works for you.
