Hello doctor,

Whenever I burp while closing my mouth, I sense a brief disturbance in my ear for two to three seconds. This does not happen all the time, only about 10 percent of the time. In a day, I experience it around four to five times.

This type of ear disturbance or pressure sensation, which occurs when you burp with your mouth closed, is most often due to transient pressure changes transmitted through the eustachian tube. When you burp with your mouth closed, the sudden pressure shift in the throat and pharynx can briefly open or stress the eustachian tube, causing a momentary popping, fullness, or disturbance in the ear for a few seconds.

The fact that it occurs only in a small percentage of burps, lasts for two to three seconds, and your hearing test is normal strongly suggests a mild, intermittent eustachian tube dysfunction (ETD) rather than any structural ear disease. Mild eustachian tube dysfunction or sensitivity usually resolves on its own and does not require specific treatment if there is no persistent pain, vertigo, continuous hearing loss, or ear discharge.

Gentle pressure-equalising manoeuvres during or after the episode, such as slow swallowing, yawning, chewing, or a very gentle valsalva manoeuvre (pinch the nose, close the mouth, and blow gently until the ears feel like they open; never forcefully).

Reducing triggers that increase burping or throat irritation, such as large meals, carbonated drinks, uncontrolled acid reflux, nasal allergy, or congestion. Consult an ear, nose, and throat (ENT) specialist if any of the following appear:

  • Persistent or increasing ear pain, continuous blocked sensation, or one-sided symptoms that do not settle.

  • New hearing loss, continuous tinnitus, dizziness, or vertigo.

  • Ear discharge.

  • Symptoms are becoming very frequent or lasting longer than a few minutes.

Before deciding on the next steps, the following investigations are advisable:

  • Tympanometry.

  • Laryngoscopy.

I would suggest the following measures:

  • Proton pump inhibitors (PPIs) such as Omeprazole or Pantoprazole, if acid reflux is suspected.

  • Eustachian tube pressure-equalisation exercises.
