Hello doctor,

I had my ear perforated due to falling into the water quickly. The doctor decided to treat a potential infection and prescribed Tarivid otic drops, tablet Augmentin 1 mg, and tablet Xyzal 5 mg so I do not have an allergy, so I will not take the antihistamine, and I want to avoid oral antibiotics and keep them as a last resort when the infection does not get treated by the topical antibiotic. Can I use the Ofloxacin as a sole treatment and increase the dosage from 2 drops twice a day which is prescribed, to 8 drops twice a day so that I can replace the Augmentin intake? Please guide. Thank you.

Yes, you can use Ofloxacin (Fluoroquinolone) drops only as a beginning. You can use it in the dose of 2 drops four times daily. Keep your perforated ear dry all the time, away from water. From when do you have this perforation? Thank you.

Thank you doctor,

The accident happened two days ago. Last night I started using the Ofloxacin and experienced lots of pain and discharge in the following hours. Thankfully, this morning's dose was fine. The notice on the Ofloxacin drops I have (Tarvid) is saying 6 to 10 drops twice daily for adults. So to confirm everything, should I use two drops of Tarivid Otic drops four times daily, then do another check-up after a week? Please help.

As the perforation is recent, I recommend minimal use of drops with the use of an oral antibiotic. The more your ear is dry, the faster the healing of the perforation. The ear drops themselves to make the ear wet. So, the use of drops four times daily can delay the healing of the perforation. I hope this helps. Thank you.

It depends on many factors. So, I suggest the oral antibiotic and the local ear drops. You can use a proton pump inhibitor for your stomach, such as Omeprazole 40 mg tablet once daily before breakfast daily for the seven days and then follow-up after a week. Thank you.
