Hello doctor,

My mother is 50 years old and she is suffering from heart diseases. Her ECG show sinus tachycardia 99, biatrial enlargement qrs area, p80 ms0, 15v1 and 0.30v2, LDS l abnormal, r wave progression early transition, qrs area 0 in v2 lvh with secondary repolarization abnormality multi, lvh criteria abnormal st t anterior st elevation probably due to lvh st 0 .20 mv in v1 v4 and lvh baseline wander in lead s v1 v2 v3 v4 rate 110 pr 166 qrsd 69 qt 319 QTC 432 axis p 75 qrs 30 t 186 12, lead standard placement. Cardiologist suggests for angiogram as she needs angiogram and she is uncomfortable with an angiogram. She has thyroid problems. She is taking BP tablet Met XL and Aspirin EC. Please suggest whether she needs an angiogram.

Firstly, you take a pic of ECG and upload. Whatever you have mentioned is machine generated report and frequently inaccurate. This is not suitable for opinion. However, from the mentioned report (attachment removed to protect patient identity), it appears that ECG has changes. However these changes may be due to hypertension alone. So she should get echo done, which will clarify the need for angiography. Next thing, if echo is abnormal and suggesting blockages then she will need angiography. If it is normal then she should get stress thallium done, which is a noninvasive test for blockages, to rule out the possibility of blockages. So overall, upload the ECG and get an echo done if not done.

You have not uploaded the ECG. Anyway, I have seen ECG interpretation by your doctor on case summary. So according to me, getting an angiography done is not a bad option but it is not absolutely necessary because there is a strong possibility that the ECG changes are due to hypertension or hypertrophic cardiomyopathy. But, we should also rule out possibility of blockages. So stress test like stress thallium or CT coronary angiography should be done, which will help to asses blockages. Continue medicines prescribed till diagnosis is confirmed. Another thing, the diagnosis mentioned on echo (attachment removed to protect patient identity) is hypertrophic cardiomyopathy which is a different spectrum of disease, which is usually not worrisome. For that, should visit a higher center to confirm the diagnosis and management. So visit nearby tertiary centre.
