Hi doctor,

I have done ECG, echo, TMT, and all blood tests. All reports are normal, and my echo showing, MVP with mild MR. I feel anxiety and stress. Please check my reports and give me your opinion, sir. I am taking Petril Beta 10 tablet. And I feel atypical chest pain for anxiety. Sir, that day you told me to take a second opinion on the TMT test, and I consulted a cardiologist; they told me all reports were normal. Please guide.

Read your echo (echocardiogram) report "Interpretation summary." There is no MR in your echo report. It is a good thing that there is no MR (mitral regurgitation) with MVP (mitral valve prolapse). The only thing you can do is take medicine to control heart rate and anxiety. Anxiety can be secondary to raised heart rate because it creates chest discomfort and fear. If Petril beta is controlling your symptoms, then continue the same. If not, I suggest switching to tablet Concor (Bisoprolol) 2.5 mg daily and tablet Petril MD (Clonazepam) 0.25 when you get anxious. Regarding TMT (treadmill test), I see changes in peak exercise. If your doctor says everything is normal after reviewing, we can trust him. We can repeat TMT after six months or in a year. Echo review to be done yearly to see any new change. Right now, do not worry; just take medicine. MVP without MR is fine; not a significant thing to do any intervention. I advise you to repeat Echo, TMT yearly. Thank you.

The underlying cause of weight loss must be treated. If it is anxiety or stress, we need to treat it. The help of a psychiatrist or psychologist can be taken. If anxiety is only due to heart rate and chest discomfort, then you need to firmly believe that nothing will happen because your tests are fine. Facing the issue and making a positive, firm belief is behavior and cognitive therapy. Weight loss causes like thyroid and tuberculosis are ruled out as your tests are normal for them. AIDS (acquired immunodeficiency syndrome) (HIV 1,2) test can also be done as this is a cause. Consult an MD medicine to rule out other causes. Improve your diet by increasing protein intake (including dietary protein supplements) and regular exercise. Take advice from a psychiatrist to deal with anxiety, and there are non-pharmacological therapies that can teach you to deal with it. MD medicine opinion to rule out weight loss causes if no improvement seen with diet. HIV and cancers are also potent causes of weight loss. In your case, it can be mostly nutritional, but other causes must be ruled out after examination. One more cause for weight loss can be parasites or worms in GI (gastrointestinal) tract. Thank you.
