Hi doctor,

I took option 72 (emergency contraceptive) 12 days after my period following unprotected sex with my partner as a precaution, although he did not ejaculate inside me. I experienced a little spotting 13 days after taking the pill, four days before my expected period date, and did not get my actual period. Now my period is five days late. Is it withdrawal bleeding or implantation bleeding? What should I do next to rule out all possibilities of pregnancy?

If you took option 72 (Levonorgestrel) pill 12 days after your period, it means most likely you took it somewhere between day 15 to 17 of your menstrual cycle. Withdrawal bleeding occurs mostly between 3 to 10 days after taking ECP (emergency contraceptive pill). If you experienced spotting 13 days after taking the ECP and have missed your period, it is highly advisable to do a pregnancy test with your first urine in the morning. If the pregnancy test comes out negative, repeat it after seven days if your period is still missed. If it is still negative, then we will start investigating other causes of a missed period.

Thank you for the reply.

To be exact, I had it on the 24th of the previous month, which is the 13th day of my cycle (my period starting on the 12th of the previous month), and the expected ovulation date was the 29th.

From the dates above, there is a 5-day interval between unprotected sex and your ovulation. The deposited sperm cells, under the best conditions, typically die within a maximum of 72 hours. The probability of being pregnant is negligible. Just be patient; your menses will come. Maintain a clear mind, as psychological stress can also worsen the delay.
