Hello doctor,

Can the ESR level fluctuate from 25 to 52 mm/hour within one day? My wife is 31 years old and had body pains, so the doctor suggested she go for an ESR blood test. Last week her ESR level report was 25 mm/hour, and the next day from another lab, her ESR report was 52 mm/hour. She is not having a cough or chest problems or any lumps, and her chest X-ray report was all right, but the doctor said that though the reports were alright, I doubted if she had sarcoidosis, so he suggested a chest CT scan. All her other blood reports are satisfactory, like complete blood count, calcium, SGPT, SGOT, etc. However, the doctor said he is still determining sarcoidosis as he has suggested Brutaflam for five days, one per day. Please suggest.

Most likely, either of the two is false. ESR (erythrocyte sedimentation rate) can rise this much, but the patient must be that serious and symptomatic, which is not our case. So it is likely a false one. It would help to repeat it from a reliable laboratory and get CRP (C-reactive protein) levels done. If sarcoidosis is suspected, then should also get serum ACE (angiotensin converting enzyme) level done. Also, these symptoms may be due to low vitamin D, so it should also be tested.
