Hi doctor,

I have a hard time reading letters on a bright white background or any other bright color for few months. I can read it, but my brain has some issues. I do not have a problem reading white letters on a black background. I was at a few doctors, but the only thing they said was that to change the power of my glasses to -6.25 left and -7.25 right. I am 33 years old, and I am a gamer. What can be the problem?

I would advise you to get a detailed retina examination done to rule out pathological myopia and wet AMD (age-related macular degeneration). Also, get an OCT (optical coherence tomography) macula done and contrast sensitivity. Get back to me with these test results.

I saw your reports (attachments removed to protect the patient's identity). There is nothing major, actually. You have a myelinated nerve fiber in the left eye. It is a congenital anomaly in which the nerve sheath is not covered with myelin which is normally present over all the nerves. You can see this in image 18 where a white band is seen below the optic disc. That is the myelinated nerve fiber. You also have a small pigment epithelial defect in the right eye. I have seen in image 5. The most common cause is smoking, tobacco, junk food, and a high-fat diet. Also, in your optic nerve test, you have preglaucomatous changes in both OCT RNFL (retinal nerve fibre layer). I advise one by one for all. Start antioxidants I-site (Lutein, Astaxanthin and L-Glutathione)/ Gloeye (Bilberry Extract and Pine Bark Extract)/ Macuvit / Macugold (Lutein, Zeaxanthin, Astaxanthin, Acetylcysteine and Melatonin) once a day for three months. No smoking, no tobacco. I would advise you to get perimetry done for ruling out glaucoma. As even if it in the early stages, treatment has to be started. Change of spectacles is a must if you are wearing less power than prescribed.Get a contrast sensitivity test also done. But the difficulty in seeing black on white is due to your pigment epithelial defect only. By increasing your power and started these medications, you will be benefited.
