Thank you doctor,

My father has cold feet and hands but he has a high fever in the range of 103 to 104. He recently had a long flight where he drank very cold water. Since then, he has a high fever. Can you please help?

  • During a fever, the peripheries may be cold due to vasoconstriction. A high-grade fever may be due to a viral illness.
  • Does he have a history of diabetes or hypertension?
  • We will need to check for:
  1. Complete blood picture.
  2. Routine urine examination including urine microscopy.
  • Considering his age, a urinary tract infection also needs to be considered.
  • For now, I suggest he take tablet Paracetamol 650 mg every sixth hourly.
  • He can take plenty of fluids and fruits.

What blood or urine tests should we get done?

  • As mentioned earlier, two basic tests such as complete blood picture and routine urine examination are to be done.
  • The other tests to be done are:
  1. Widal test.
  2. Dengue serology Ns1 antigen.
  3. Dengue IgG.
  4. Dengue IgM.
  5. Smear for Malarial parasite or PV/PF card test.

Ask him to check for any eschar present over the body. An eschar is like a small healed ulcer.
