Hello doctor,

I am a 32-year-old male who had foot surgery in September. I am non-weight bearing and on bed rest, which has caused severe anxiety, panic attacks, dizziness, and restlessness. I have tried talking to family and meditation but found no relief. My healthcare provider prescribed Clonazepam temporarily. I consulted a psychiatrist who prescribed Olanzapine at night and Fluoxetine for anxiety. I have concerns about side effects and need short-term relief. Is this the best medication, and what can I do for immediate relief while waiting for Fluoxetine to take effect? Kindly help.

I read your query and understood your concern. You can take one capsule of Fluoxetine 20 mg daily in the morning and a tablet of Clonazepam 0.25 mg twice a day. The Clonazepam will provide relief until the full effect of Fluoxetine sets in. It takes about four weeks for the full effect, but improvement should start within one week. Fluoxetine is among the best and safest antidepressants, although it might cause some gastrointestinal issues. Also, I do not think you need to take Olanzapine. I do not have a history of fearfulness, hallucinations, or suspiciousness. My panic and anxiety started after my surgery, and I feel helpless. Can I just take Fluoxetine and skip Olanzapine? Are there other medications or combinations you would recommend in my case? Kindly suggest.

I read your query and understood your concern. I do not suggest taking Olanzapine for anxiety alone, as it is an antipsychotic medication. I suggest you take one 20 mg capsule of Fluoxetine daily in the morning after breakfast. For daily morning anxiety, take one tablet of Clonazepam 0.25 mg after breakfast. If needed, take an additional 0.25 mg in the morning. Take one tablet of Clonazepam 0.25 mg at night. If sleep disturbance persists, you can take 0.5 mg that is two tablets. However, if you encounter issues, you can use Cyproheptadine 2 mg and skip the Fluoxetine capsule on days when you are sexually active.
