I think these are fordyce spots and are harmless ectopic sebaceous glands only. These can be seen over the genitals or lip mucosa. The cause of these is not very well known, but as these are completely harmless occurrences in many people, no treatment is usually required. This will not lead to any problem in any way, be it sexual or otherwise.

And these are not a part of any STD (sexually transmitted disease).

Can Fordyce spots occur on the foreskin?

I am just wondering because these spots are on the foreskin where we do not sweat. Also, sometimes, there is a burning sensation in these spots.

What about lichen nitidus? Can it be that?

Fordyce spots are very common over the foreskin (prepuce).

Very rarely, they can have mild burning, but yes, they are largely asymptomatic.

Lichen nitidus is more of a skin-colored dome-shaped lesion, mainly present over the shaft of the penis diffusely, not localized to the foreskin.

Clinical appearance does not favor lichen nitidus.

I can state facts about why it is not lichen nitidus, but there is no point in doing that much here as these are clearly Fordyce spots.

There is nothing to worry about in this condition. Just apply a moisturizing cream like Cetaphil and clean with a mild cleanser like Cetaphil cleanser for sensitive skin to avoid occasional burning.

And these do not sweat glands. These are enlarged sebaceous glands (oil glands).

I have collected only a few images of Fordyce spots over the same area from the internet. Fordyce spots are more common over lip mucosa and then genitals. Thisbeing a common condition, I do not usually take pictures of it in my practice.
