What is the treatment for Fordyce spots on the upper lip?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have noticed Fordyce spots on my upper lip vermillion. Prior to starting adapalene on my face and head, I did not have any bumps on my lip. However, these bumps emerged after using 3 % Adapalene for approximately six months. My dermatologist was uncertain about their nature and initially treated them as eczema, but they have not subsided and can sometimes become irritated. My current medications include Klonopin 4 mg for PTSD and Protonix 40 mg. In the past, I was prescribed fluticasone propionate cream for the same complaint. I am not satisfied with the presence of these bumps and I would like my lip to return to its normal appearance. Additionally, I have also developed some hyperpigmentation on the top lip. Kindly help resolve my issue.

Hello,

Thank you for stopping by at icliniq.com.

I am here to provide expert opinions on your medical issues. I have carefully reviewed the photo (attachment removed to protect the patient’s identity) and the details that you provided. Based on the information provided. Your condition does not appear to be herpes and seems more consistent with fordyce spots. You may have only recently noticed them. Usually, no treatment is necessary for fordyce spots unless desired for cosmetic reasons. If you want, you can undergo laser treatment or other cosmetic interventions to remove them. If you believe the appearance of the spots coincided with the initiation of adapalene, discontinuing adapalene may be worth trying, especially if it has served its purpose. It is important to note that hyperpigmentation or sun tan can be potential side effects of adapalene usage. For prevention of further hyperpigmentation, I suggest you avoid sun exposure and use sunscreen. I suggest you apply Fluticasone cream only if there is irritation or as needed. I suggest you continue to follow up with your dermatologist for further management and guidance on addressing your concerns. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.
