How do I treat panic attacks, and anemia due to heavy menstrual flow other than birth control pills and antidepressants?

Hello doctor,

I have had panic attacks and anxiety attacks. Also, I have periods almost every day, resulting in my blood iron level dropping, because of which I have a high pulse rate and feel fatigued all day, and I am in bed all the time. Also, I get diarrhea and stomach aches whenever I take iron supplements and take Cyclizine. I also tried taking Sertraline, but it did not help with the condition. Therefore, I do not want to take birth control or antidepressants as the medication for this condition. Kindly help me with some medicines for the same.

I have gone through your query and understand your concern. Your symptoms suggest iron deficiency anemia, for which iron infusion therapy is good. For high pulse and anxiety, I would suggest beta-blockers, and no need to take an antidepressant. And for the periods, I would suggest oral contraceptive pills that will not cause depression.

Thank you for your reply.

Some of my friends take oral contraceptives, and when enquired with them, they often feel depressed and have mood swings. I was prescribed a progesterone pill I need to take until I want to start a family. However, I am in my twenties and do not want to take progesterone pills daily. Also, my blood test reports show low iron levels. I have also attached MRI (magnetic resonance imaging) scan reports. Which contraceptive do you suggest? Is there any other alternative for progesterone pills?

I would suggest a combined oral contraceptive pill, Depo-Provera (Medroxyprogesterone acetate) injection, or intrauterine devices. Also, you can take iron or vitamin D supplements. And for the stress, I would suggest beta-blockers.
