Hi doctor,

My son has gastrointestinal tract issues. He has diarrhea and stomach pain all the time. He got blood and biopsy tests done, and we need help interpreting them. Please help us. Thank you.

I can understand your concern. The biopsy reports have no significant positive findings. Just mild acid reflux and gastritis are suggestive. I suggest you get a stool examination done and calprotectin tests (which will help determine if any inflammatory bowel disease is present). A stool test for lipase and also for reducing substances is suggested. Your child may be unable to digest certain food ingredients. Thank you.

Hi doctor,

Thanks for the response. He did take these tests. His lipase levels are 44 U/L, stool calprotectin is 31 μg/mg, and RBC (red blood cells) is low. His immature absolute granulates were off, and carbon dioxide was high. Thanks and regards.

Irritable bowel syndrome (IBD) is ruled out with low calprotectin. If you can share the complete blood count report, I can help better. High immature granulocytes suggest an infectious or inflammatory etiology. Also, I am concerned about stool lipase and not serum lipase. Please reply to me with answers to the following questions. Are the stools pale? Is there any blood present in the stool? Was the microscopic examination of stool done? What is the duration of symptoms? Thank you.

Thanks for the response. I have attached the reports. They are pale stools. I have seen blood a few times. His stomach pain will last for 20 minutes, a few times a day. Thank you.

I went through the reports (attachment removed to protect the patient's identity). There is no significant abnormality with the blood work done. Is your child losing weight as well? A diagnosis of celiac disease should also be considered. In addition, gluten sensitivity could be the issue. It would help if you also tried restricting fat from the diet. We need to move in a stepwise manner. Stool microscopy will also be helpful. You did not mention the duration of the symptoms. I will insist on performing another important test to rule out celiac disease. The tissue transglutaminase IgA (immunoglobulin A) test is the preferred celiac disease serologic test. Diarrhea might be due to malabsorption, and it indeed is one of the common causes. Thank you.
