Hi doctor,
I am a 50-year-old man who has been dealing with GERD for several years. Recently, I have started experiencing a loss of appetite and a persistent bad taste in my mouth (almost metallic), and I have noticed yellowing of my skin and eyes.
Could these symptoms indicate liver-related issues or a more severe complication of GERD?
Should I consider liver function tests or upper GI imaging?

The metallic taste in your mouth is likely due to gastric acid backing up into the esophagus, leading to reflux esophagitis (stomach acid flows back into the esophagus). Upper gastrointestinal imaging is indeed useful in assessing the extent of tissue damage caused by gastroesophageal reflux disease (GERD). A barium swallow test may also be helpful, especially if you are experiencing difficulty swallowing.
The yellowish discoloration of your eyes and skin could be a sign of elevated bilirubin, often associated with liver issues. However, some medications like Amoxicillin or Aspirin can also cause this symptom. If you do not have associated itching, there may not be an immediate cause for concern. If the discoloration persists for several days, it may be worth getting liver function tests.
