Why do I feel heaviness in my head after a headache?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

Last week, I had a headache on the right side of my forehead and right eye. I took Paracetamol for some duration; the pain was ok at night. I could not sleep much.

The next day, I checked blood pressure; it was 125/80 mm of Hg (millimeters of mercury). I went to an eye consultation and got my eyes checked; everything was fine. Then I went to a general physician; there, my blood pressure came to 140/98 mm of Hg.

He asked me if I was stressed. I told him I have anxiety problems and even overthink for long. He gave me Cipro LA 40; the pain stopped after starting the medicine.

Now it has been five days, and I have been advised to take it for ten days and call after that. But I do not have a headache, but there is a little heaviness in my head and discomfort in the back of my neck sometimes, and early on, there was dizziness. Checked blood pressure three to four days at home; it was normal.

What to do for this little heaviness I feel in my head?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I am deeply concerned about your worries.

Thank you for reaching us.

So based on your history, your symptoms are most consistent with a tension-type headache associated with anxiety and poor sleep, along with a temporary stress-related rise in blood pressure.

The fact that your scans and eye exam were normal, your home blood pressure readings are now within normal range, and the headache improved after starting Ciplar (Propranolol) is all reassuring.

The heaviness in the head, lightheadedness, and neck discomfort you are feeling now are very common after a recent episode of anxiety and headache. They are often due to muscle tension (especially in the neck and scalp), lingering stress, and sometimes the effect of propranolol, which can make some people feel slightly tired or lightheaded in the beginning.

At this stage, there is no sign of anything serious. The focus should be on recovery and stabilizing your routine:

  1. Maintain regular sleep (this is very important).

  2. Stay well hydrated.

  3. Do gentle neck stretching and consider warm compresses.

  4. Limit excessive checking of blood pressure, as this can increase anxiety.

  5. Continue the medication as prescribed and follow up with your doctor.

Try not to focus too much on the sensation of "heaviness"; this often persists for a few days to weeks after a stress-related episode but gradually improves as your body relaxes.

You should seek medical review only if symptoms become severe, persistent, or are associated with warning signs like vision problems, vomiting, weakness, or imbalance. Otherwise, what you are experiencing is benign and reversible, and with better sleep and stress control, it should settle completely.

I hope my answer is clear for you, and please do not hesitate to ask any question at any time.

Do follow up whenever needed.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

It has been five days already since taking Ciplar LA 40; the heaviness in my head made me more worried, and I had neck discomfort.

Does this medicine have such an effect for so many days?

Also, the first-time blood pressure reading was high, so the hypertension also made me worried. I am trying all healthy lifestyle options, maintaining weight, and eating healthy. Sometimes worry makes me bring some anxiety with me.

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I am deeply concerned about your worries.

Your symptoms are most consistent with a benign tension-type headache related to anxiety and poor sleep, and there is no sign of anything serious. The heaviness in your head, mild lightheadedness, and neck discomfort are common in this situation and can also occur during the first days of taking Ciplar (Propranolol) as your body adjusts to the medication.

A single high blood pressure reading during stress does not mean you have permanent hypertension, especially since your home readings are normal, which is reassuring.

The main issue here is the anxiety cycle, where worry increases muscle tension and makes symptoms feel worse. The best approach is to continue your medication as prescribed, improve sleep, stay hydrated, do gentle neck relaxation exercises, and avoid repeatedly checking your blood pressure.

These symptoms are temporary and should gradually improve as your stress and sleep normalize.

I hope my answer is clear to you, and please do not hesitate to ask any questions at any time.

Do follow up whenever needed.

Let me know if I can assist you further.

Thank you.
