Patient's Query

Hi doctor,

I am 47 years old. I was suffering from urinary retention, heartburn, and fever for five years. I went to a doctor, and he referred me to a urologist. I had a culture examination and sonar. It showed an infection, and there were problems with my urine, including nitrates and protein. He gave me some medicines, but no improvement. Then, I was referred to a cardiologist, and the blood pressure was found to be high. I had an echo test, and the result of the echocardiogram showed the presence of a muscle pressing on the heart. He gave me two medicines, Colosar-Denk50/12.5 and Bisocor. The medicines were effective, and the result was positive. But after a while, the Colosar-Denk50/12.5 medicine was no longer available, and I continued to use only Bisocor.

Two years back, I was very sick and went to a cardiologist, and he gave me the previous medicines. This year, one of the medicines was not available, and I changed medications, which caused me problems such as fever and pain in the head and chest. I revisited the doctor, and he told me you need a doctor with more experience than me. The doctor forbade me from eating animal fats. Now, I want to know your cardiologist's opinion about my condition.

What is the appropriate treatment?

Your echocardiography (Attachment removed to protect patients identity) is normal. It says diastolic dysfunction but does not tell about which grade of dysfunction. It has not given values. Your LDL (low-density lipid) is on the higher side. It is bad cholesterol. The first medicine given to you was an antihypertensive. Probably, your BP was high.

I suggest checking blood pressure two times in the morning and two times in the evening for one week and writing it down to see average blood pressure. If it is high, then antihypertensives will be continued. If BP is higher, you should also take Rosuvastatin 10 mg tablet daily, which lowers cholesterol. Bisocor is a medicine that decreases heart rate. Usually not given as first-line antihypertensive.

Please check your BP, have HbA1c levels done for sugar levels, and have a repeat ECG done if possible so that your condition can be managed accordingly.
