Hello doctor,
I am a 27-year-old male, and I am seeing small spots on the gland on my penis. There are red spots that go away and come back. I am having pain while urinating and itching on the first day, which will go away after that. I had done blood tests for (herpes simplex virus) HSV-1 and HSV-2, which came positive. I am currently under Acyclovir 400 mg three times daily. Please help.

I read your query and understand your concern. Herpes is a recurrent disease, and Acyclovir 400 milligrams three times daily is a minimal dose. It should be taken at least six to eight times daily for a long-term effect. I opened both files, and minor red-headed dots are visible (attachment removed to protect the patient’s identity).
I advise you to wash the genital area gently with Povidone iodine solution mixed with warm water at least three times a day. Apply Fusidic Acid ointment, Sulfamethoxazole and Trimethoprim ointment, and Silver Sulfadiazine with Neomycin ointment in a 1:1:1 ratio gently. Over that, sprinkle Fluconazole powder. It helps reduce itching and discomfort.
The treatment includes continuing Acyclovir six times a day for seven to ten days, then reduce the dose. But complete the full seven to ten days course. Consult a specialist doctor, discuss with him or her, and take all medications with their consent.
Take Deflazacort 12 milligrams two times a day after lunch and dinner for six days. Also take Montelukast Sodium and Levocetirizine Hydrochloride, one tablet at night for fifteen days or more, or as needed. This combination is a very good antiallergic.
Take Fluconazole 150 milligrams once daily to prevent any fungal infection. Again, please consult a specialist doctor before starting or continuing these medications.
For your kind information, herpes can still recur. Whether it comes back or not depends on the body’s immunity. To improve immunity, you can add Vitamin C (from citrus fruits), Vitamin B complex, a general multivitamin capsule for males, cod liver oil pearls, and protein powder with milk two times a day. These supplements are generally safe even for long-term use.
