Hi doctor,
My wife is 34 years old and has increased ferritin level after two iron infusions. She has a low CRP, with no evidence of overload in heart and liver, cancers, or hemochromatosis. Two hematologists said that they did not anticipate a problem. She took blood CRP sediment test, MRI, blood test for hemochromatosis, genetic test, liver ultrasound, endoscopy, and colonoscopy. She is vaccinated and got booster dose for vaccination. Is she prone to COVID?

Increased ferritin indicates iron bone marrow storage level. It is natural to have high ferritin temporarily after iron supplement. If she acquires COVID-19 (coronavirus disease), then ferritin level and CRP (C-reactive protein) can go high because it is acute phase reactant due to severe inflammation. In omicron variant clinical improvement is fast and the patient need not check their ferritin or CRP.

My wife has IUD to stop heavy menstrual bleeding. This helps to maintain iron in blood. But I am concerned about her increased ferritin level of 1900 mg/L. She took a break from work. Can you tell the severity of COVID?

Based on ferritin level future COVID infection severity cannot be predicted. Ferritin is one of the inflammatory marker, so it is used during COVID infection as well to predict the severity. But now high ferritin could be due to iron infusion only. There is an increased risk factor if she has underlying conditions like diabetes, hypertension, kidney abnormalities, or abnormal liver function. Omicron does not cause death compared to delta variant. Repeat ferritin test after one month.
