Patient's Query

Hi doctor,

How many days before should a person start PrEP (Pre Exposure Prophylaxis) to build immunity against HIV before having sex with high-risk groups. Also, is there any ointment to use in the scrotum for protection against genital fluids during sex? How effective is PrEP with the medicine Tenvir-EM?

Hi,

Welcome to icliniq.com.

I understand your concern. Pre-exposure prophylaxis does not build immunity against HIV (human immunodeficiency virus). It only attempts to kill any of the viruses that enter your body during any such exposure. Also, pre-exposure prophylaxis is available only through prescription. You must take the drug daily until there is continued high-risk behavior and not miss any pills. A combination of Tenofovir and Emtricitabine is the preferred choice and provides 99 % protection.

Patient's Query

Hi doctor,

Thank you for the reply.

Suppose one will indulge in high-risk today. So, how many days before one should take Tenvir-EM? Also, is there any ointment to use in the scrotum for protection against genital fluids during sex?

Hi,

Welcome back to icliniq.com.

I understand your concern. It is better to start at least one day before. There are no ointments, but you could use a condom.

Patient's Query

Hi doctor,

Thank you for the reply. Kindly review my medical report. Herpes is found positive. How infectious is it?

Hi,

Welcome back to icliniq.com.

You have not attached any report. Kindly attach the appropriate report.

HSV (herpes simplex virus), IgM (immunoglobulin M), or IgG (immunoglobulin G), which one is positive?

Patient's Query

Hi doctor,

It is already attached but sending it again.

Hi,

Welcome back to icliniq.com.

I understand your concern.

Have gone through your report (attachment removed to protect the patient's identity). IgG for HSV is positive, which suggests that there was an infection in the past for which your body has developed immunity. IgG are protective antibodies and will help to prevent future infections. There is no treatment required unless you have any active symptoms of herpes.

Patient's Query

Hi doctor,

Thank you for the reply.

So is it possible to have oral and protected vaginal sex and lip kisses with the infected one? How dangerous is herpes as STD? What are the chances of transmission from the infected one to a healthy person where the infected value is more than 9 IU/ml?

Hi,

Welcome back to icliniq.com.

I understand your concern. Herpes is a common infection, and most people develop immunity to it in their lifetime. Herpes can affect the genitals and oral cavity usually. However, in rare cases, it can even occur at sites of contact with the infected area. The major issue with herpes is recurrent episodes, which require early initiation of treatment. Prevention of the spread of herpes requires proper usage of a condom. You can participate in sexual activities with IgG-positive herpes patients as they are non-infective and have developed immunity. With regards to IgG number, the more, the better.

Patient's Query

Hi doctor,

Thank you for the reply.

I have uploaded the blood report from another laboratory. Kindly comment on the herpes level and about having intercourse with the infected one.

Hi,

Welcome back to icliniq.com.

I understand your concern.

IgG is positive, which suggests immunity has developed. You can participate in protected sexual activity if there are no active blisters over the genitals or oral cavity.

Patient's Query

Hi doctor,

Thank you for the reply. But will it spread to me if there is protected sex and no active blisters over the genitals or oral cavity?

Hi,

Welcome back to icliniq.com.

I understand your concern. If there are no active lesions, then it means there is no contagious infection, and test results suggest you developed immunity. So there will not be any spread of infection. Always use a condom.
