Hi doctor,

My virgin girlfriend and I had our first intercourse two days ago, and she started experiencing pain. She had severe pain, that she had to lie down on the floor to ease. When I asked her, she said it was more than her menses pain. What could be the problem?

When a lady's hymen ruptures, that is, after the first sexual exposure, there will be few tears in and around the external genitalia and also the vaginal wall. These will be the point of entry of minor infections already present in the normal vaginal flora and also in the semen. This will be more if you have not used a condom. These infections will enter the uterus and thereby into the pelvis or lower abdomen, causing cramps on and off, similar to menstrual cramps. You do not need to worry. All you need to do is avoid sex for a few days until the pain subsides fully and have a short course of any antibiotics like tablet Azithromycin 500 mg daily once for five days or tablet Levofloxacin 500 mg daily once for five days. Use any of the antispasmodics (usually used by women during periods for pain) like Dicyclomine, 2 to 3 times per depending on the severity of pain. Use a condom during sex.
