Hi doctor,

I have been suffering from low blood sugar for more than six months. I have episodes of sweating, tiredness, and shaking, which are resolved when eating something sweet. After tracking my levels, it seems to be within a few hours of eating that it drops. Blood sugar can be between 48 mg/dL and 61 mg/dL during these periods. I am under Citilopram now. I have had blood tests done, and I am not diabetic, and everything has come back clear, including my thyroid.

You may have reactive hypoglycemia. I suggest a mixed meal test - baseline glucose, insulin, and C-peptide every 30 minutes for the next four hours after a mixed meal (usually a mix of a protein drink) or a 75-gram glucose solution. For managing, try eating complex starch, mixed meals (smaller carbohydrates, high fiber, moderate protein, and fat), and avoid all liquid calories. Exercise regularly and try losing some weight. Other things like Cortisol and growth hormone deficiency are to be considered. A 72-hour fast test may be needed. Please attach blood tests. Do you have other symptoms like weight loss, body pain, nausea, vomiting, darkening skin, craving for salty or sugary food, etc.? Have you visited an endocrinologist so far? You need a face-to-face consult either directly or by video chat to consider if any other tests need to be done.

Thanks for the reply.

I am currently on the waitlist to see an endocrinologist, so I am looking for some answers on the cause and how to control it. I do not have access to blood results, but I was told everything was clear and normal. I go to the gym six days a week. I have no weight loss or pains etc. Just these episodes when my sugar drops and then I remain out of sorts or tired the rest of the day, and they can happen as much as more than five times a day.

Talk to your general practitioner and maybe place a continuous glucose monitoring sensor, and you can manage your day better. Try doing the tests I suggested before you visit the endocrinologist. Glucose, insulin, and C-peptide - fasting every 30 minutes after a mixed meal or 75 g glucose. To manage, try eating complex carbohydrates mixed with generous protein and fat and no liquids except water. Continue working out at the gym. Again having a CGMS (continuous glucose monitoring sensor) will be very useful.
