Can hypothyroidism cause infertility and depression at 29?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 29-year-old woman feeling exhausted all the time. My periods have become very irregular, and I have gained weight despite eating healthy. My TSH (thyroid-stimulating hormone) came back at 8.5 microunits per milliliter, and free T4 (thyroxine) is slightly low. They said I have hypothyroidism. Please tell me;

  • Will I need to take thyroid pills forever?

  • Can it cause infertility or depression in women if left untreated?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

Based on what you have described, it does sound like you are experiencing primary hypothyroidism, and your symptoms align closely with that diagnosis.

What do your lab results mean?

A TSH (thyroid-stimulating hormone) of 8.5 microunits per milliliter is above the normal range (typically 0.4–4.5), indicating that your brain is signaling your thyroid to produce more hormones.

A low free T4 (thyroxine) confirms that your thyroid is underactive and not producing enough hormone, which your body needs for energy, metabolism, and overall function.

Are your symptoms related?

Yes, the symptoms you are experiencing are very typical of hypothyroidism. These are;

  1. Fatigue and low energy.

  2. Irregular or skipped periods.

  3. Weight gain, even with a healthy diet.

  4. Often also includes dry skin, cold intolerance, constipation, and brain fog.

Will you need thyroid medication forever?

You will likely need to take Levothyroxine, a synthetic form of thyroid hormone. Whether it is lifelong depends on the following factors;

  1. If it is autoimmune hypothyroidism (like Hashimoto’s thyroiditis), then yes, you will probably need lifelong treatment.

  2. In rare, temporary cases (like postpartum thyroiditis or after a viral illness), thyroid function may return, and medication could be stopped.

  3. But most adults diagnosed with hypothyroidism due to an underactive thyroid will require long-term or lifelong replacement therapy. The medication is generally well tolerated, safe, and inexpensive.

Can hypothyroidism cause infertility or depression?

Yes, if left untreated, hypothyroidism can cause several issues, like;

  1. Because thyroid hormones are important for ovulation and maintaining a healthy menstrual cycle, their imbalance can cause infertility

  2. Low thyroid hormone affects neurotransmitters and may cause depression.

  3. The good news is that treatment with Levothyroxine corrects these problems in most people once the dose is optimized.

What should you do next?

You can follow the following steps to keep yourself healthy;

  1. Start thyroid hormone replacement as prescribed.

  2. Get TSH and free T4 rechecked about 6 weeks after starting treatment to adjust the dose.

  3. Take the medication on an empty stomach, ideally in the morning, and avoid calcium or iron within 4 hours.

  4. Talk to your doctor about checking thyroid antibodies (TPO-Ab) if the cause hasn’t been clarified.

  5. If you are planning pregnancy in the future, it is important to keep TSH in the normal range (ideally below 2.5 during conception and early pregnancy).

You have to understand that hypothyroidism is common, especially in women in their 20s and 30s. Your energy levels, weight, cycles, and mood can improve with proper treatment. Starting thyroid medication is often the right step in the right direction.

I hope this information helps you.

Feel free to ask further queries.

Thank you.
