Hello doctor,

I have a yellowish-white film growing all over the skin, and sometimes it has black spots in it. It made me lose my hair, and all of my nails look infected. When I peel it off, it is almost like my hair comes out of it? It is very weird. My children have it too. Any idea what it could be? Could this be yeast?

After going through your history, I want to get some details from you. Did you get this infection from your kids or the other way around? Was there any red itchy sore before the yellow crust? After looking at your pictures (attachment removed to protect patient identity), you might be having a condition called impetigo. It is a type of bacterial infection and can spread among family members. Close contact with another person with impetigo is the most common risk factor for illness.

The Probable causes

The probable cause is bacterial infection by streptococcus or staphylococcus.

Investigations to be done

Do gram stain and culture from the skin lesions.

Differential diagnosis

The differential diagnosis is psoriasis.

Probable diagnosis

The probable diagnosis is impetigo.

Treatment plan

Apply antibiotic ointment Mupirocin 2% over the skin lesions twice a day for one week.

Preventive measures

Clean all minor cuts and injuries that break the skin (like blisters and scrapes). Wash clothes, bedsheets, and towels every day and do not share with anyone. Cover your mouth and nose with a tissue when you cough or sneeze. Wash hands with soap and water for 20 seconds. If there are any deep ulcers visit your doctor.

Have a follow-up after one week with reports.

I tried Mupirocin ointment and had no results. This yellow stuff covers every single pore on my body, and it is very uncomfortable. We all got it at the same time, but I noticed my symptoms first. I have also used medicated shampoo from a dermatologist, and again I had no results. It is thick too. But my son's penis is covered completely, and you can see yellow around it. It looks like pus that wants to come out.

According to your history, you have used Mupirocin but got no relief. So it may be due to some other cause. But to make a confirmed diagnosis, I suggest you have a dermoscopy done or a histopathological examination of the skin. For that, you need to visit your dermatologist and get a piece of diseased skin tissue removed for microscopic examination. Once the diagnosis is confirmed, it would help you to get a targeted treatment of your condition.

The differential diagnosis are allergic contact dermatitis, psoriasis and dermatitis artefacta.
