Hello doctor,
I am a 32-year-old female. Both the ovaries are polycystic. My husband is 33 years old and has erection issues. The doctor suggested IUI, but the husband's motile sperm count was only 10 million. The doctor advised going for IVF with ICSI. Can I go for a single embryo transfer? I also have a 1.5 cm intramural fibroid on the posterior wall of the uterus. What is the chance of pregnancy with a single embryo transfer? Please give me your opinion.

The sperm count of 10 million is more than sufficient for IUI (intrauterine insemination). Please go for IUI, and you have an excellent chance to conceive. So, do not worry. You are around 9 kg overweight (attachments removed to protect patients identity), so please try to lose excess weight. It will help your egg quality to become much better. Even if you lose 5 kg, it will help you a lot. The fibroid should not interfere with conception. I suggest you take your husband to a urologist to find out the cause of his erection trouble. It will be more helpful than anything else. For further information, consult an infertility specialist.'

Thanks for your response.
Concerning my earlier mail would like to mention was put on Dubagest 300 mg and told to take a urine test. For the past two days, I have been feeling mild cramping in the lower abdomen, sometimes very mildish but sharp pains on both the left and right sides of the lower abdomen but not simultaneously. I am very, very worried if I am suffering from ectopic pregnancy. I have some more queries. If I go for IVF with ICSI, can I transfer a single embryo? Do I need to take bed rest in case of IVF pregnancy? My husband has erection issues, which could not be solved with medicines, and also, no test of him has been found negative except for low motile sperm count. Is there any life threat for me?

I suggest you maintain that the sperm count is more than adequate for Intrauterine insemination (IUI). Regarding ectopic pregnancy, to find out if u have it, you need to do a blood test, namely the serum beta HCG (Human chorionic gonadotropin ) test to find out. It needs to be done every 48hrs. Whether or not you can have a single embryo transfer depends on your finances and the quality of the embryos. You don't need to be on bed rest if you get pregnant, whether by Ivf or otherwise. it depends on several factors in the pregnancy itself, some of which can't be predicted.

Could you please refer to my earlier reports? I did IUI with a 10 million motile sperm count. i was suggested one dose of Dubagest 300. I want to know about my course of Dubagest as I am bleeding. Could it be an early miscarriage? My last menstrual period was a month back, and how can I have a period again?

In my opinion, it is unlikely to be an early miscarriage, most probably, and it is your menses. Please do the test I suggested in the previous consultation, that is the serum beta HCG, to be sure of the same.

Thank you for your response.
My Dubagest dose is still on. How can I have a period now? And period happens when progesterone level falls, right? Why so much pain and cramping and watery blood suddenly in the mid of the night. Sorry to say, but I made the mistake of doing clitoral stimulation yesterday even though very gently, to which I had an orgasm and after that, mild pain started. Would you please give me your say in this?
