Hello doctor,

I am a 30-year-old and have been diagnosed with internal hemorrhoids, and I have bled almost every day for two months. Now it has subsided and the bleeding occurs scantily and occasionally from past a month. I had severe abdominal bloating issues due to acidity, and it subsided.

I was having severe diarrhea, and I met a gastroenterologist who prescribed Rifagut (Rifaximin) 400 mg for 15 days, Flagyl (Metronidazole) 400 mg for 10 days, Econorm (probiotic) for 20 days, and Daflon (Diosmin, Hesperidin) for 15 days.

My stomach pain increased after that, but my loose stools have stopped. I took another opinion from a physician who told me that these symptoms are mainly due to severe iron and vitamin B12 deficiency, and it would give me a feel-good feeling once I get treated for the same.

Recently, I started experiencing widespread body pain related to bones and muscles. I feel very sensitive too. I am extremely worried, and your insights into my health will be greatly appreciated.

I am attaching my blood test reports for your reference.

I have seen your reports (the attachments have been removed to protect the patient’s identity). You have iron deficiency anemia. The underlying cause of iron deficiency anemia is bleeding that occurred for two months due to hemorrhoids.

If your piles are not resolved and lead to much bleeding, then consult a general surgeon if the operation is needed. You may be given an antibiotic (Rifaximin 400 mg) and a probiotic, which are usually prescribed for gastroenteritis and the treatment of Bacterial overgrowth syndrome.

The treatment for iron deficiency anemia includes:

  1. Iron sucrose injection can be prescribed to increase hemoglobin levels for 5 days.

  2. Oral iron medication for two to four months to replenish iron stores, under monitoring.

Anemia decreases the oxygen-carrying capacity of blood, so such symptoms can occur.
