Hello doctor,

I am scheduled for a laparoscopy to diagnose and treat my underlying conditions. Over the past six months, I have had irregular periods with heavy bleeding, spotting, and discomfort during ovulation. I also experience vaginal bleeding during bowel movements, bloating, and a constant feeling of fullness, making me uncomfortable to wear my usual clothing. Recently, I started experiencing severe right-sided pelvic pain, radiating to my back and top of my thigh. An emergency doctor suggested it might be a cyst or endometriosis and prescribed Cocodamol 30 mg and Ibuprofen while I await the operation.

However, I developed slight swelling in my right foot and an achy, tight calf. A&E ruled out DVT, but they advised expediting my operation due to the severity of the pain. Despite leaving messages for my consultant, I have not received a response, leaving me feeling helpless and overwhelmed. I have a history of PCOS and currently take B12 injections, Cocodamol 30 mg, and Ibuprofen. Kindly help.

It appears that the swelling over the ankle on the same side indicates that the pain could be attributed to a possible lymphatic obstruction in the pelvis, which may be secondary to endometriosis adhesions. Considering your symptoms, endometriosis is a likely cause. Therefore, it is advisable to expedite the diagnostic laparoscopy, and adhesiolysis should be performed if necessary. Following the procedure, your doctor may recommend the use of GnRH analogs to manage endometriosis.

For pain relief, the current option available is NSAIDs. You might consider using cyclooxygenase inhibitors like Celecoxib (available with a prescription), which have been shown to provide better pain relief.
