My 48-year-old mother has hypothyroidism and is bleeding for two weeks. Please advice.

My mother is 48 years old. She is having irregular spotting. She missed her period last month. She consulted a doctor and did some blood tests. Her sonography was normal. But her TSH was high around 17 mIU/mL and she is suffering from hypothyroidism. The doctor advised her to take Eltroxin 50 mg. Now she is having bleeding with low to medium flow for the past two weeks. Please advise.

Kindly share her scan reports. Thyroid abnormalities can cause some bleeding irregularities. As your mother is in perimenopausal age, she can have irregular cycles due to anovulatory cycles. As she has bleeding for two weeks, it is good to take hormonal medications to control bleeding. She needs to take some supplements to improve her hemoglobin levels.Your mother can take tablet Livogen Z (Ferrous fumarate+Folic acid+Zinc sulfate) daily once after taking food for a month. She can also take tablet Trapic MF (Tranexamic acid with Mefenamic acid) or Pause MF thrice daily after food while having a heavy menstrual cycle. She can continue taking Eltroxin 50 mg (Thyroxine) daily on empty stomach after brushing her teeth, Primolut N 10 mg (Norethisterone) thrice daily till her bleeding stops. After that, she can take Primolut N 10 mg once daily for the next 14 days and then stop taking them completely. Your mother can get her periods after stopping them. It is good to avoid missing pills in between and it may lead to irregular bleeding again. She needs to reduce her weight, do regular exercises with diet modifications. She should avoid taking goitrogenic foods such as cabbage, cauliflower, broccoli, radish, and soy products. If the bleeding persists, she needs to have an endometrial evaluation.
