What causes irregular periods and blood during masturbation?

Hi doctor,

This is a little awkward. My period changed a little. I did not pay much attention to it because I heard it was normal. Recently (around two months ago), I missed my period for the first time ever. The next month was alright, and this month I missed my period again. I have also noticed a small amount of blood during or after I masturbate, which is also unusual for me. I have never had sexual intercourse before, so I am worried about what might be going on.

There could be many causes of irregular periods and intermenstrual bleeding. It would be best if you had investigations and precautions and then treated accordingly.

1. Sudden increase in weight or decrease in weight, habits and environmental change, any illness, medications, contraceptives, strenuous exercise, strict diet, thyroid abnormalities, and hormonal imbalance can delay periods.

2. Check for the above factors and abnormal hair growth as well as increased BMI (body mass index). Irregular periods and abnormal hair growth are signs of PCOS (polycystic ovary syndrome).

3. Do regular exercise, yoga, or walking to reduce weight and have a balanced BMI.

4. Eat a healthy, nutritious diet with less salt, sugar, oil, and spice.

5. Do recreational activities and quit recreational drugs because they will harm your fertility.

6. Do a cervical smear test, a high vaginal swab, and one local outpatient hysteroscopic examination by a gynecologist to exclude any cervical or endometrial polyp or any other local pathology or trauma.

7. Transvaginal scan is also needed.

8. You can have tablet Norethisterone (synthetic hormone) 10 mg twice daily from day 5 to day 24 day of your menstrual cycle to have regular periods and prevent unscheduled bleeding for three months.

9. On day two of periods, have serum testosterone, serum progesterone, serum prolactin, serum insulin, FSH (follicle stimulating hormone) and LH (luteinizing hormone), thyroid function test, and complete blood count tested.

10. Collect all results of laboratory test results and discuss them here or with your gynaecology local specialist.
