Hi doctor,

My wife has itching and peeling on her feet, especially when she wears shoes throughout the day. She has had this issue since childhood. She tried tablet Hydrocortisone, but it did not help. How to manage?

Your wife's symptoms indicate plantar keratoderma. Ask her to avoid chronic friction or ill-fitting footwear and ask her to wear soft sandals. Ask her to apply Liquid Paraffin lotion for 21 days. Ask her to avoid injury or trauma at the site. Ask her to avoid scratching and rubbing the lesions. I suggest (consult with a specialist doctor, talk with him or her and take medicines with their consent) tablet Loratidine 10 mg once daily after breakfast for a week if itching is present for her. Ask her to avoid over-the-counter steroids as there is no inflammation. Steroid causes dryness in the skin and results in peeling. Ask her to moisturize the skin daily after bath for 20 days. Ask her to stay hydrated and drink 7 to 8 glasses of water daily. Follow up after 21 days.
