Hello doctor,
My children and I are experiencing itching all over our bodies. The itching is worse in the morning and less during the day. When we scratch our skin, a white substance appears on the surface.
I have taken injections, even antibiotics. What could it be?

When multiple family members, including children, develop generalized itching, especially when it is worse in the early morning or at night, this pattern is highly suggestive of a contagious skin condition, most commonly scabies. Scabies is caused by microscopic mites that live in the upper layer of the skin and spread easily through close contact within households. Antibiotics and injections usually do not help in such cases, which explains why symptoms may persist despite treatment.
The itching in scabies is typically intense and often worse at night or early morning because the mites are more active during rest. The “white material” that appears when scratching is usually dry skin scales or superficial skin debris, sometimes mixed with serum from irritated skin, and does not rule out scabies. In children, scabies can involve the whole body, including the trunk, hands, feet, and sometimes the scalp, while adults often have involvement of the fingers, wrists, waistline, armpits, and genital area. Sometimes visible rashes are minimal, but itching can still be severe.
For proper confirmation and guidance, it would be very helpful to share clear photographs of the affected skin areas from both adults and children, taken in good lighting. This helps differentiate scabies from other causes such as eczema, dry skin, or allergic conditions. Laboratory tests are usually not required if the clinical picture fits
General care is very important while treatment is being planned. All family members must be treated at the same time, even if someone has mild or no symptoms; otherwise, reinfestation occurs. Clothes, bed sheets, towels, and blankets used in the last few days should be washed in hot water and dried in sunlight or a hot dryer. Nails should be kept short, scratching should be minimized to prevent infection, and regular use of a gentle moisturizer can help soothe the skin after treatment.
Please note that itching may persist for two to three weeks even after correct treatment, due to an allergic reaction to dead mites, and this does not mean treatment failure. Until diagnosis, take tablet Cetirizine 10 mg at bedtime for adults and syrup Cetirizine 1 tsp (teaspoon) twice a day for kids above two years old. Consult your specialist doctor, discuss with them, and take the medicines with their consent.
