Patient's Query

Hello doctor,

I have had a persistent dry and itchy skin patch on my calf for about eight years, and it occasionally bleeds when scratched. Recently, I have noticed other smaller dry skin patches, but none are as severe as this one. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

Based on your description and the provided picture (attachments removed to protect the patient's identity), it appears to be lichen simplex chronicus, a condition characterized by skin thickening due to repetitive scratching or rubbing. It can affect various areas, and in your case, it involves the flexor aspects of the knees and calves. This condition may result from dryness, venous insufficiency, or weakened blood vessel walls, possibly aggravated by prolonged standing. I suggest using xerina cream (purified water, Glycolic acid, Urea, Cetylated fatty ester complex, Ammonium hydroxide, Glycerin, Glyceryl monostearate, PEG-100 stearate, Cetyl palmitate, Propylene glycol, Stearic acid, Cetearyl alcohol, Tocopheryl acetate, Isopropyl myristate, Oleo European oil, Polyacrylate-13, Polyisobutene, Polysorbate 20, Simethicone, Perfume, Methyl paraben, Disodium EDTA, Propyl paraben) or moisturex cream (10% w/w Lactic Acid, 10% w/w light liquid paraffin, 10% w/w Propylene glycol, and 10% w/w Urea), waiting for 15 minutes, and then applying a topical steroid with salicylic acid, such as Propysalic NF lotion, twice daily for two to four weeks. Additionally, I suggest taking one tablet of Levocetirizine 5 mg once daily for a similar duration can help alleviate the itching. I suggest you consult your general practitioner or a nearby dermatologist before starting any medication. It is essential to get a follow-up review after one month or as necessary.

I hope this helps.
