Hello doctor,

My sister's AMH is 0.7 ng/mL. She is 40 years old and trying for her second child. She conceived her first child naturally ten years back, but now she has one failed IVF and six failed IUIs, and spontaneous egg rupture before egg retrieval. But she is still trying for a second child. Will she be successful if she tries for her second IVF with an AMH of 0.7 ng/mL? Does she has any chance of natural conception?

The normal level of AMH (anti‐Mullerian hormone) is 1.5 to 4 ng/mL. The lower level can be 1 ng/mL to have a conception.

If we talk about how naturally she can improve her fertility or egg reserves at the age of 40, that can help her very little, but better to have those tips as well.

1. Fish, high protein diet, food with DHEA (dehydroepiandrosterone), more fibers, nuts, seeds, and fresh HDL (high-density lipid) oils to be taken on a long-term basis.

2. Daily exercise, walking, and yoga for having normal BMI (basal metabolic rate) and weight.

3. Recreational activities to have better emotional health and have to quit recreational drugs if she or her partner used to take them as these not only reduce fertility but also can cause congenital anomalies.

4. Have folic acid daily.

Due to age, it is better to have IVF (in vitro fertilization) with hormonal support at every step. It could be ICSI (intracytoplasmic sperm injection) or other methods to have pregnancy as soon as possible. As I advised above, it is better to start work immediately, have urgent consultation with a fertility clinic, and start a healthy workup.
