I have pain in left buttock, and sore left leg after walking. Please review my ultrasound report. I am currently on cholesterol, Aspirin, and blood pressure.

I have seen the report (attachment removed to protect patient identity).

It has occluded posterior tibial artery which is the cause of pain in the leg after walking. Also, there is a possible stenosis or block of the stent in common iliac vessels which is the possibile cause of limping.

Doppler or duplex report is just a suggestion to confirm the above-mentioned finding CT (computed tomography) angiography of the lower limb is mandatory. Continue the same treatment.

I will advise you one more drug to start. Please go through CT angiography so further management decisions can be planned.

I have attached earlier medical records. There are the notes of the vascular surgeon concerning the procedure of stenting and angiogram previously. Current medicines are Rosuastatin, Cilazipril, Terrosin (for BP), Aspirin, Pregabalin (peripheral nerve pain in both feet), and Tramadol (damaged knee).

Current symptoms are pain in left leg (in the buttock) the whole leg becomes sore (not just calf muscle) shortly after beginning a walk. If I stop walking pain eases. These symptoms are very similar to previous problem in 2014 largely resolved by stents and angiogram. I was able to walk for 8-10 km per day very easily. Not any more. Current symptoms came on quite quickly (maybe over one month). What other tests will help to diagnose? What has happened to my leg? What do I need to fix the problem?

I can understand your concern. As per your previous report and current symptoms, this is a case of stent blockage. That is why symptoms reappeared. Also, there is a block in the tibial artery of the leg which is the reason of pain during and after walking.

Your medicines are fine. No need to change or modify but the problem is all the medicines are for prevention. Once a new block is there, then there is no use of medication.

Now the only treatment is a fresh angiogram, look for the block, removal of blocked stent, and placement of a new stent. This is the only treatment available as of now if you have symptoms. For all of these, you have to contact your vascular surgeon again.

Same symptoms don't mean you have the same problem. Consult a doctor now!
