Hi doctor,

I have pain on the left side near the pelvis. It started five days ago while I was doing stretch exercises. The pain was so severe I had to lie down. I had some blood work done which showed no abnormalities. I also got an X-ray which showed arthritis. I have been taking the tablet Advil to reduce the pain. I woke up to increased pain this morning after a good sleep. I wonder if there is something more than arthritis, like an internal organ problem.

From your description, I firmly believe the sharp pain is due to some ligament stretch, and the pain gets aggravated in a specific position. Kidney stones cause unbearable pain on the side under the ribs and radiating to the groin. Rest for a few days; you can continue taking the tablet Advil (Ibuprofen) and add a muscle relaxant like the tablet Zanaflex (Tizanidine)10 mg thrice a day for a few days. Consult a specialist doctor, discuss with them and take medications with their consent. The problem with ligament strain is that it will not have enough blood supply, so it takes a long time to recover fully, sometimes two to three weeks. Let me know if you have any questions.

I do not think it is from internal organs. Internal organ pain is different, dull, primarily constant, not associated with exercise, and has some other symptoms, like pancreatitis with nausea and vomiting. So do not worry, but if it does not get better in one or two weeks, see your doctor for a physical examination.

In the middle of the night, I woke up feeling the same sharp pain in my left pelvis. Would it be if I had a ligament stretch that would suddenly cause pain when sleeping? I could not get into a comfortable position. The pain only subsided by taking a tablet Advil and applying heat.

Moving or sleeping on one side can be a reason. If it doe not get better within days, see your doctor in the office. Still, this is not visceral organ pain. Let me know if you have any burning sensation or abnormal discharge while peeing.
