Hello doctor,

A known friend of mine has been suffering from liver cirrhosis and portal hypertension for the past two days and complains of fever with chills, and has slight yellow eyes. The fever is still present. PCM was taken for fever, but the effect was temporary. We consulted a gastroenterologist who had prescribed medications, but there was no relief from the fever, and still, my friend has yellow eyes.

Please let me know whether this could be some kind of serious issue or will it heal slowly. Also, let me know whether this condition is due to some problem in the liver or is due to something else. I have attached the necessary details and prescription. Please go through it and let me know.

From your reports (attachments removed to protect the patient's identity), the patient's condition does not seem to be serious. His liver is compromised and is not functioning properly. The reason is due to long-term alcohol intake as mentioned in one of the reports (attachments removed to protect the patient's identity) that you have uploaded.

I think he needs a regular follow-up with the gastroenterologist which he is already doing but I also suggest you take a second opinion from another gastroenterologist preferably in person. Once the liver gets damaged, it is unable to recover without a liver transplant. But liver transplant is not needed at the moment, but he will definitely need one in the future. You must consult a transplant surgeon and a transplant physician to decide the right time for liver transplantation. I need to talk with the patient before offering the best advice with which he can proceed.
