What is the sufficient sperm count for pregnancy?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been married for the past three years, but my wife has experienced a miscarriage and has not been able to conceive since then. Our sex life was fine initially but for the last year, it is not that good. I am losing interest in sexual intimacy with my wife, and despite spending most of our time together, we only engage in sexual activity around three to four times a month.

For the past 18 years, I have been masturbating daily or every other day. It has become my hobby to masturbate and not have sex with my wife. Frankly speaking, I enjoy masturbation more than physical sex with my wife. In the initial months of marriage, I felt good having sex with my wife. I now find myself less sexually attracted to her, sometimes even feeling attracted to others. She sometimes complains about not having regular sex, although she herself seems disinterested at times.

I recently had semen testing and my total sperm count was 50 million/ml, 70 percent active, and 6-8/HPF pus cells. The semen test was done after abstaining from sex for five days. I had a serum testosterone test and it was 265. I consulted a doctor and was prescribed the following medicines: Cavenor sachet (twice daily - for one month), Androwin (once daily - for three months), and Tazzle 5mg (once daily - for half a month) for erectile dysfunction. I do not have any other diseases like diabetes, high blood pressure, thyroid, etc. These are some specific queries that I have in this regard.

1) Is it common to lose interest in sex after two to three years of marriage?

2) What is the role of the Cavenor sachet in this case?

3) Whether current medicines are fine or will you suggest any others?

4) Does ED go away or start again after stopping medicine?

5) I took Tazzle 5 mg once and felt a buzzing sound in my left ear for one day. After that, I stopped taking that medicine. Is this a side effect of medicine like Tazzle?

6) We do not have sex daily due to a lack of interest from my side or her side. Can a medicine like Tazzle be taken every day or only if you intend to have sex on that day?

7) Is a 50 million/ml sperm count enough for pregnancy? How many days to abstain from sex before the semen test?

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

It is not uncommon to lose interest in sex after two to three years of marriage, although libido may be lower compared to the initial period. Tablet Cavenor, which contains L-arginine, may help improve blood flow and could be beneficial. The current medication appears to be satisfactory, but the combination of medications depends on various factors. Erectile dysfunction (ED) can sometimes resolve if it is psychological in nature.

Maintaining physical health through a healthy lifestyle and avoiding harmful substances is important. Tablet Tazzke may have certain side effects. Tablet Tazzle can be taken daily in low doses or used as needed. For semen analysis, a minimum of three days of abstinence is required.

I hope I have answered your question.

Let me know if I can assist you further.

Regards.

Patient's Query

Hi doctor,

Thank you so much for the reply. I have some follow-up queries in this regard.

I took Tazzle 10 mg once and stopped after the ear buzzing sound. A few days later I took Tazzle 5 mg once, and stopped again after an ear buzzing sound. It has been 10 days since I last took that medicine. I feel some ear buzzing sound still in my left ear. I am continuing to take Cavenor sachet, and I feel it creates interest in having sex daily.

1) Is there any damage to my ear due to taking Tazzle?

2) How much time will it take before the ear buzzing sound goes? Should I take any medicine for my ear now?

3) Can you suggest any alternative medicine for erection purposes that has no side effects?

4) Is Cavenor sufficient to cure ED without taking a tazzle in this case?

Looking forward to your reply.

Thank you.

Hi,

Welcome back to icliniq.com.

I can understand your concern.

It is unlikely that Tazzle (Tadalafil 10 mg) causes permanent damage. It may take a few days to a few weeks to get complete recovery. Cavenor (L-arginine and Proanthocyanidins) may be helpful if taken regularly for one to three months. Exercise regularly, and avoid alcohol and tobacco. Follow a healthy diet.

Hope this helps you.

Thanks and regards.
