I have a tonsil stone without fever or pain. How to treat it?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have a yellow tonsil stone measuring 3 × 3 mm, without fever or pain. It has been there for more than a week. I feel a lump in my throat and dryness in my throat. I am taking anxiety medication (Nexito 2.5 mg).

Please advise.

Thank you.

Hello,

Welcome to icliniq.com.

I completely understand your concern.

After reviewing your reports (attachments removed to protect the patient’s identity), here is what we have found: there is a lump in the upper back part of your mouth, specifically in the soft palate area. You have mentioned feeling discomfort, especially with swelling and throat dryness, and this has been present for about 10 days. There is no fever or pain, and I also note that you are currently taking anti-anxiety medication.

Based on the findings, the provisional diagnosis in your case is swelling of a lymph node in the soft palate region (part of Waldeyer’s ring). Other possible causes (differential diagnoses) could include general inflammation and swelling, or tonsillar tissue growth in the midline of the soft palate.

For treatment, the plan may include:

  • Antimicrobial medications are used if there is an infection in the lesion.
  • Anti-inflammatory medications to reduce swelling.
  • In some cases, surgical removal of the affected lymph node (if it involves Waldeyer’s ring and does not resolve with medications).

The overall prognosis appears to be fair. In the meantime, please maintain good oral hygiene.

I would also recommend undergoing some additional tests, such as a swab smear, radiographic imaging, and possibly a biopsy if required. Culture and antimicrobial sensitivity tests may also help guide treatment.

Once you have the test results, please share them with me so I can guide you further.

I hope this helps clarify your concerns.

Thank you.
